Here's the full report.  One of my favorite audit checks; the GAO identified over 1,800 dead people receiving controlled substance prescriptions.

The technology exists today to routinely monitor for the type of fraud and abuse identified.  If physicians knew that someone was helping them identify fraud & abuse then true patients just might find it easier to achieve adequate pain relief.  I am more concerned about these patients than the people who abuse the system.
